This notice explains the narrow business information collected through the US Lab Partner website and the boundaries of the public inquiry workflow.
Information collected
The inquiry workflow collects business contact information, laboratory identity and location, selected production categories, approximate volume, timing, workflow preference, source information, and an optional general production note.
Information this website must not receive
Do not submit patient names, dates of birth, prescriptions, scans, identifiable clinical photographs, live case files, patient-linked case numbers, protected health information, payment-card information, banking information, or tax documents. No file upload is offered on the public site.
How inquiry information is used
Information is used to review commercial fit, respond with relevant wholesale or test-case next steps, coordinate laboratory onboarding, manage follow-up, and maintain an operational record of the business relationship. It is not a live-case record.
Storage and access
Inquiry and onboarding records are stored in the website’s protected operational database. The management view requires sign-in and an explicit owner allowlist. Authorized records may be exported for a legitimate CRM handoff.
Correction, deletion, and contact preferences
To correct business information, request deletion, or stop follow-up, email cases@uslabpartner.com. A direct “no” response will be treated as a do-not-contact request.
